Working to Make Your Child Better | Overview

 

Some of our research projects involve working to better our clinical practice. We feel it’s imperative to stay involved with innovative networked initiatives to help us take the best possible care of our patients from when they are first diagnosed to when they are ready to transition to adult care.

ImproveCareNow

ImproveCareNow is an active, open learning health network that uses collaboration and data to drive improvements in the care and health of all kids with Crohn's disease and ulcerative colitis. Since ImproveCareNow began, the percentage of kids with Crohn's disease and ulcerative colitis who are in remission (feeling well, with no symptoms and fully active) has increased from 50 percent to over 75 percent — all without new medicines.

Got Transition?

Got Transition? is the National Health Care Transition Center that aims to support optimal transitions from pediatric to adult models of health care for youth with and without special health care needs (www.gottransition.org). Got Transition? will facilitate the implementation and dissemination of health care transition best practices in primary care medical homes and specialty settings for youth and young adults, especially those with special health care needs.
