The NCCCTA is housed within the Integrated Care Program at Boston Children’s Hospital and directed by Dr. Richard Antonelli. It is a partnership with the National Center for Medical Home Implementation (NCMHI) in the American Academy of Pediatrics. The NCMHI is supported by the Health Resources and Services Administration (HRSA) of the United States Department of Health and Human Services (HHS) grant number U43MC09134.
