We ask that a legal guardian or the patient (if 18 years or older) complete our request an appointment form. All patients must complete our intake process before we can schedule in our program.
- Step 1: Patients/families complete our intake form, which includes questions about demographic information, past medical and mental health history, and current symptoms.
- Step 2: Our team will request the patient’s previous medical records and growth history from the primary care office if needed. This information may also be faxed to 617-730-0184.
- Step 3: Our medical team reviews these intakes once a week. During these sessions, we determine medical severity, prioritize medical urgency, and determine if our outpatient program can provide appropriate care.
- Step 4: After this meeting, one of our program coordinators will reach out with updated information and to discuss next steps. If our medical team determines that you are not eligible for our program, we may also make recommendations for alternate programs and/or resources.

- Please allow up to 10 business days for review after submitting.
- If it has been longer than 14 business days since you submitted your intake forms/questionnaires and you have not received notification that we have received them, please email ArfidEDIntakes@childrens.harvard.edu and let us know.
