Requesting a Neuropsychological Evaluation

The Neuropsychology Program provides neuropsychological evaluations for children with medical and neurological disorders. To request an evaluation, please call us at 617-355-8434.

For evaluations for children with attention or learning problems, please call the Developmental Medicine Center at 617-355-7025 or the Learning Disabilities Program at 617-355-2868.

For evaluations for children for autism spectrum disorders, please call the Autism Spectrum Center at 617-355-7493.

For evaluations for children followed in the Epilepsy Center, please call 617-355-7970.

Intake forms

We require all families to fill out an intake form in order to be added on to our waiting list. You can print out the form below or complete the form electronically. Please return completed paperwork via mail, email, or fax to:

Please note: We will review the intake form to determine the appropriateness of our program to address the needs of the child/adolescent. We will contact you after we receive the intake form. If it has been longer than two weeks since you submitted your intake forms/questionnaires and you have not received notification that we have received them, please call 617-355-8434 and let us know.
