Medical home services

We are currently working toward becoming a certified medical home. A medical home is not a place – it is an approach to providing comprehensive primary care that facilitates partnerships between patients, clinicians, medical staff, and families. We share the belief of the American Academy of Pediatrics (AAP) that every child deserves a Medical Home, where care is accessible, continuous, comprehensive, patient- and family-centered, coordinated, compassionate, and culturally effective.

At the core of a Medical Home is a knowledgeable, compassionate health care provider and care team chosen by a patient and their family to take care of a child/youth’s health needs. 

At Brookline Pediatrics, we have a dedicated Medical Home Care Coordinator, Danielle Spencer. Her role is to work in partnership with the pediatric care team at Brookline Pediatrics and the children and families in the practice to ensure that both the medical and non-medical needs of our patients are met. Danielle takes an active role in helping us to coordinate the care of our patients with complex medical history or special health care needs and ensures that all of our patients receive comprehensive, quality care.  She also provides resources and helps to establish connections between patients, the practice, and specialists, schools, and services in the community.
