Appointments
Please arrive 10 to 15 minutes ahead of your scheduled appointment.
Bring your insurance card and co-payment to every visit.
Cancellations: Broken appointments represent a cost to us, to you, and to other patients who could have been seen in the time set aside for you. Cancellations are requested 24 hours prior to the appointment. Post Road Pediatrics reserves the right to charge for missed or late-canceled appointments. Excessive abuse of scheduled appointments may result in discharge from the practice
School and camp forms
We require five to seven business days to complete your child's forms. Your child must have an up-to-date annual physical (within one year). If you are dropping off a form, please include your child's name and date of birth on the form. If medication orders are needed, please include the names of the medications your child will be taking at school/camp. Thanks!
