Started in 2007, the Critical Care, Anesthesia Perioperative Extension (CAPE) and Home Ventilation Program is a comprehensive and longitudinal service for children with chronic respiratory insufficiency, technology dependence and related medical complexities. The program promotes high quality, coordinated care to assist families and improve the lives of children with long-term ventilator assistance. CAPE services are tailored to the individual patient, offering extensive home visits, outpatient clinics, and inpatient consultation to meet a wide range of needs.  Our providers are also engaged in health services research and interventional studies to improve care systems and outcomes.

CAPE enables families and children to receive, and provide, complex medical care in your community, while promoting your child’s growth and development.

Supplementing the Medical Home

CAPE Program physicians and staff specialize in caring for children with complex healthcare needs. We work closely with primary care providers to create models of care that are tailored to individual family and practice needs.   With primary providers in the community, we work to create patient-centered medical homes for children with a wide range of complex problems.  These can include complications of prematurity, organ transplantation, neuromuscular disease (e.g., spinal muscular atrophy and muscular dystrophy), acute neurologic injury, and many other critical illnesses.

What do we offer?

CAPE providers help determine if, and how, short and long-term respiratory support can be of benefit. Services are intended to optimize your care at home and facilitate any hospital-based care, augmenting the Medical Home / primary care services:


  • Long-term primary respiratory/pulmonary care and healthcare coordination with families and primary care providers with 24 hours a day response
  • Consultation and support to families who are considering homecare of children requiring noninvasive respiratory care or transtracheal ventilator assistance
  • Critical care follow-up for short term or temporary respiratory support as well as sedation and analgesia weans
  • Transitional/discharge care planning to coordinate with community services, rehabilitation facilities, and other acute care hospitals
  • Home ventilator equipment and environmental safety evaluations
  • Interface with home nursing, school/therapy staff, and equipment services
  • Inpatient collaboration with medical and surgical specialists
  • Consultation services for at risk patients when planning critical care recovery and rehabilitation
  • Consultation and outreach to long-term care facilities for families not living in the community
  • Social work support with assessment of psychosocial, financial & medico-legal issues, short-term crisis intervention counseling, and case management
  • Collaboration with palliative care services
