Division of Gynecology | Your Visit

Our location

We are located at 333 Longwood Avenue on the fifth floor, across the street from the main entrance of the hospital. There is a Bank of America on the first floor of the building. (Please note, we also provide gynecology services at three of our satellite locations in Lexington, Peabody, and Weymouth).

Parking is available in Boston Children's Hospital parking garage one block away on Longwood Avenue. Bring your parking ticket with you, as we will validate it for a reduced rate.

What to bring to your visit

For an easier check-in, bring the following information with you on the day of your appointment:

  • your insurance card
  • your insurance co-payment
  • any outside paperwork from your primary care or referring doctor
  • referral information/authorization

Please arrive 15 minutes before your scheduled appointment to complete the administrative check-in process.

New patients

If you are a new patient to the clinic, please print it out and complete the Medical History Questionnaire before your appointment. Make sure to bring this form with you on the day of your appointment.

Insurance information


Copayments, deductibles, and any coinsurance are due at the time of your visit per insurance company policies. A copayment is required for each scheduled appointment. Please check your insurance card for amount due. We accept personal checks, VISA, MasterCard, American Express and Discover.


A referral is a form or code number from your child's primary care physician or insurance company and is required for office visits with most HMO insurance plans. Referrals can vary in the number of visits they cover, the length of time that visits can be used and the services covered.

If your insurance company requires referrals, you will need to obtain them. Note, referrals do not guarantee coverage, so please check directly with your insurance carrier.

Waiver fee

If you do not have a valid referral from your primary care provider (PCP), Boston Children's policy requires a guarantor to sign a waiver assuming all responsibility for payment of the visit.

Insurance card

Please remember to bring your child's insurance card to the office for each visit, so we can verify that we have the correct information. If you change insurance companies or if your policy number has changed, notify the office immediately so we can update our records.

Uninsured/non-covered services

Uninsured patients or services not covered by insurance companies may be eligible for a 40 percent discount due at the time of check-in. Contact our financial coordinator to discuss this option at 617-355-2536.

Additionally, if you are uninsured or have a limited income, you can contact our Financial Counseling team at 617-355-7201.

Ultrasound and MRI information

If you are scheduled for an ultrasound or an MRI prior to your appointment, proceed to the Radiology Department on the second floor of the main hospital.

If you are having a pelvic ultrasound, you will need to have a full bladder and should drink 32 ounces of water prior to the test. Proceed to our office immediately after completing the test with the preliminary report, which will be given to you by the technician.
