We are very excited to have you join us.  Please feel free to contact us any time with questions or concerns you might have.  We take pride and joy in providing the highest quality medical care in the most personal and efficient manner.  Before you come in for your first visit with us you may print out and complete the forms below. We would also like to remind you to bring your insurance card with you.

Prenatal visits

We are happy to schedule a prenatal visit with expecting parents.  You will have the opportunity to come into the office and sit down with the doctor to ask any questions you may have.

Baby’s first visit

After you and your baby are discharged from the hospital we will see you and your newborn within the first few days.  Please bring the hospital discharge papers and your insurance card with you to your first visit.

Patient Portal

On the Patient Portal, you can do a number of things such as updating or adding to your demographic information and surgical information, view your child’s immunization record, request new appointments, and cancel or view upcoming appointments.  In addition to this, we can send reminders for upcoming appointments.

If you would like to sign up to use the Patient Portal, please stop by the office to pick up a form or print out the form and send it to us through the mail.

Parent Connection

Parent Connection is a time when our patient’s parents can get together along with one of our doctors and discuss a specific topic.

For example, the topic of our first meeting was ADD/ADHD. So for parents of children with ADD/ADHD, parents who have children who were recently diagnosed, and anyone who is interested in this topic can come together and discuss the hardships and the successes of having a child with ADD/ADHD. By communicating with other parents you can learn new tips and ways to help your child who is diagnosed with ADD/ADHD. We encourage our parents to come and share their experience and exchange information.
