We continue to hold Covid-19 vaccine clinics for 5-11 year old patients. Please check MyChart for continued updates on clinic dates. Scheduling will be done on MyChart ONLY. Please do not call the office to schedule.

  • Patients 5+ years old are eligible for the Pfizer vaccine. All patients 18+ are eligible for the Moderna and J&J vaccines
  • If you have received the vaccine and have not yet contacted our office with records, please email pnw466@gmail.com with the date(s) of your vaccine and a photo of your vaccination card.
  • As of January, 2022, booster doses are approved by the CDC for the following groups:
    • Booster shots of the Moderna and Pfizer Covid-19 vaccines are available to immunocompromised patients 4 weeks after your 2nd vaccine. More information can be found here.
    • Booster shots of the Pfizer, Moderna or J&J vaccine are available to all patients 12 years old and older.
