Notes:
The MMR/Varivax (MMRV) is a combination vaccine given at 4 years of age.
The DTaP/IPV (Kinrix) is a combination vaccine given at 4 or 5 years of age.
Gardasil is a 2 dose series if started before 15 years old and 3 doses if started at 15 or old.
**Flu vaccine is recommended yearly starting at 6 months of age.
