PACC Program
Hyde Park Pediatrics believes in the medical home philosophy and we strive to provide the best care possible to all of our patients and their families. As part of this effort, we have a program called the PACC (Pediatric Alliance for Coordinated Care) Program.
This program was initiated by Children's Hospital to assist pediatric practices to care for the most complex patients in their own community. The PACC Program, run by Debra Simons, one of our nurse practitioners, is for select families who have children with complex medical needs.
The program enables us to provide extra resources and oversight to help with management of complex medical issues, specialist appointments, durable medical equipment, letters of medical necessity, medication review, and any other support that is needed. Helping families with these concerns and crystallizing each medical issue enables better and more comprehensive care of these patients which hopefully leads to healthier outcomes and decreased family stress.
