Pain-related Pediatric Assessment Tools

Fear of Pain Questionnaire, Child and Parent report (FOPQ-C; FOPQ-P)
Questionnaire to assess pain-related fears among children and adolescents living with chronic pain.

Parent Fear of Pain Questionnaire (PFOPQ)
Questionnaire to assess parent pain-related fears of children and adolescents living with chronic pain.

Parent Pain Acceptance Questionnaire (PPAQ)
Questionnaire to assess parent acceptance of their child's pain.

Chronic Pain Acceptance Questionnaire for Parents (CPAQ-P)
Questionnaire to assess parent perceptions of their child's pain acceptance.

English adaptation of the PHODA-Youth in collaboration with Jeanine Verbunt, MD and Marielle Goosens, PhD at Maastricht University, Netherlands, who created a Dutch version of the measure.

Pediatric Pain Screening Tool (PPST)
Pediatric adaptation of The Keele STarT Back Screening Tool in collaboration with Jonathan Hill, PhD at Keele University. The PPST is designed to identify a patient’s risk status (high, medium, or low) of poor pain-related outcome using established psychosocial prognostic factors. The validation for this tool was recently published and can be accessed here

Pediatric Camp Outcome Measure (PCOM)
Questionnaire to assess the impact of the therapeutic camp experience among children and adolescents living with a chronic illness.

Parent and Adolescent Medication Barriers Scales (PMBS; AMBS)
Questionnaires to assess barriers to medication taking from the parent and adolescent perspective. Initially developed for use with pediatric transplant recipients, but has been disseminated across several populations of children living with chronic medical conditions.

Multidimensional Adherence Classification System (MACS)
Medication adherence classification system that incorporate self-report and drug assay levels to incorporate subjective and objective measures of adherence data to better classify adherence among pediatric transplant recipients.
