Completing this form does not guarantee your reservation. Once your application has been received, you will be contacted by program staff.
You can download an application and fill it out electronically. Once you have completed the form:
- click on the "File" menu
- select "Save As"
- rename the document using your name (for example, "smith-application.doc").
- You can then fax completed applications to (617) 919-3451.
Or, you may email it as an attachment to familyhousing@childrens.harvard.edu. Applications pasted within the email will not be accepted.
