Course Description

Boston Children’s Hospital faculty will present successful state-of-the-art collaborative care models that promote meaningful interactions between primary and subspecialty care providers and patients/families. This innovative approach—one utilized by Boston Children’s and its community-based primary care affiliates—provides an integrated model of care for patients with common or chronic neurological, gastrointestinal, brain injury and cardiac needs.
