Course Requirements |

Course Requirements include:
|

|
- Letter from the applicant(s) Chief or Director stating that his/her home institution is your sponsor, including curriculum vitae and interested dates of observership
- Course duration is 2 weeks; there is a non-refundable fee of USD $3,000.00 per two weeks; four week stay, USD $4.500.00 (meals and housing fees are not included and non-reimbursable)

Applicants meeting course requirements will receive:
1. Letter of acceptance and Hospital required forms
2. Letter for immigration authorities to obtain visa (upon request)
3. List of housing possibilities only
4. Upon course completion, certificate of attendance.

Maximum number of four participants at any given time.
If applicant is accepted, payment is due ten days prior to arrival date and check(s) can be made payable to CHMC Cardiovascular Surgical Foundation.
