Scheduling

Call our new patient coordinator toll-free at 1--888-Pedi-Onc (733-4662). The coordinator will request the following materials:

• Pathology slides, from initial diagnosis to current date, with written reports
   
• Imaging studies with reports (CD-ROM or DICOM-compatible preferred; hard copy films also accepted)
   
• One- to two-page medical summary letter, including patient's diagnosis, treatment to date and questions from provider and/or patient
