When canceling an appointment, please give us as much notice as possible, at least 24 hours in advance, so that we may accommodate other patients.

Name and telephone number of your referring physician
Co-payment for office visit (if applicable)
Any forms mailed to you by our office
Any previous imaging (MRIs) you might have
Your Child's Exam
You will meet with a neurologist on your first visit. He or she will review your child's medical history and perform a specialized physical exam. First visits usually last one hour.
Follow-up appointments
Depending on your child's condition, you may also need to meet with other specialists in our clinic. This usually requires a separate appointment, which can be scheduled before you leave. Tests such as a brain MRI or EEG may be scheduled as well.
