Martha Eliot Health Center

Services

I am an adult patient at Martha Eliot.  How do I transition my care to a new provider?

Your MEHC primary care team will have more information as to which centers are accepting patients and you can discuss this at your next scheduled visit. If you do not have an appointment scheduled, please schedule one as soon as possible to discuss this directly with your care team.  We are planning to transition all adult patients by June. You may also call the helpline at 1-855-281-5730 to call to discuss your questions, concerns and needs.

What services do the clinics at Martha Eliot Health Center provide?

We are no longer accepting new adut patients, however, we will continue to provide care to children and adolescents at Martha Eliot.  Our health care team provides medical, nutritional, mental health care and optometry services.
