Initial appointments at any of our locations can be made by you, your referring physician, or your primary care physician.

Please call your preferred location for an appointment.

Please have the following information available when you call:

• Reason for the therapy
• Insurance information including insurance type (HMO, PPO, POS, etc.), group and/or member ID number, and subscriber name
• Name, address, and telephone number of your referring physician
• Name, address, and telephone number of your primary care physician
• Your home, work and cell phone numbers, and email address
