To make an appointment with Dr. Mayer at Perkins, please call 617-972-7296.
After scheduling the appointment for you child, the Perkins Low Vision Clinic scheduler will send you the forms for:
- Insurance Referral
- Permission to release medical information on your child
- Directions for getting to the Perkins Center.
If Dr. Mayer has seen your child before, tell the scheduler that she/he is an old patient from her practice at Children's Hospital.
