How to make an appointment

• Call 617-355-7796. Press 3 to schedule an appointment.
• Press 1 to schedule an appointment for a patient who has been assigned to a physician.

Or...

• Press 3 to schedule an appointment for a patient who has not been assigned to a physician.
   

The scheduler will ask for all necessary information regarding your child, including demographics, medical history and Insurance. Once that is completed there are several options that vary by diagnosis/complaint, which include:

• If no testing is required and you have a specific urologist in mind, then you will be scheduled by that physician's scheduler.

• If no testing is required and there is not a specific urologist that you want your child to see, then the information is given to our nursing coordinator, who will assign your child to a doctor.

• If testing is required and has been done, the scheduler will request that those results and possibly films be sent for your child's doctor to review so that if additional testing is needed, it can be scheduled at the time of the initial visit.

• If testing is required and has not been done, the scheduler will get the assigned doctor's request and schedule those tests for the initial office visit.

Team of experts
Nurses and physicians at Children's have access to one of the largest resource centers for pediatric care. Should your child require involvement of other specialists, his or her pediatric urologist will arrange for a referral and a follow-up appointment. We try as much as possible to coordinate care among specialists and will do everything possible to set up appointments on the same day.

Comforting, supportive environment
The length or type of hospital admission (outpatient procedure or inpatient admission) will depend on the procedure or care your child needs.

This double sided form informs your clinician of your child’s medical history which is imperative to the initial evaluation of your child. Please bring the completed and signed sheet with you during your child’s initial visit to the Department of Urology.

When should patients/guardians complete this form? 
Complete this form if it is your child’s first visit to the Department of Urology, or if you haven’t been seen in the Department of Urology in more than 3 years.

Special Instructions:
Under the “Medical History” section on page 2, if the diagnosis’s do not apply, please check the ‘normal’ box.

Authorization to Release Information and Assign Insurance Benefits
