Coming to the Mitochondrial Program

Before your visit to our Mitochondrial Program at Children’s Hospital Boston, we will ask you to send medical records from any previous evaluations your child may have had at other facilities.

We also ask that you take time to fill out the questionnaire we will send you. This form will help us better understand your concerns and questions and your child’s medical history.
