How do I make an appointment?
Before we can schedule an appointment, the finances MUST be settled. We require a prepayment equivalent to half of the parent responsibility as well as a signed Guarantor of Payment form before an appointment can be made. Payment can be made by check, Mastercard or Visa. If applicable, a signed school contract must also be received.
Parents, as Guarantors, are ultimately responsible for the entire bill.
Once an appointment has been made, the Program Coordinator will send you parent and teacher forms to be completed before the evaluation.
