As your surgeons, our goal is to provide you and your family with the highest quality care possible. Before your visit, please take time to gather relevant health information about your child to fill out the General Health Form as completely as possible. You may download the form from the link below or request that it be mailed to you when you make an appointment.
You can help by bringing the following items to insure the best care for your child:

Appointment Information:
Physician's Name
Department (Neurosurgery)
Building Name/Location
Hospital card
Imaging studies (X-rays, MRIs and CT scans) and radiology reports
Insurance cards
Insurance referrals if applicable
Other helpful items to make your visit more enjoyable:
Books to read while waiting (there are books and magazines available in the waiting room as well)
Snacks
Formula, diapers or a change of baby clothes.
Before You Leave Your Child`s Appointment:
Request all school notes/forms that need to be completed
Request all prescriptions for medications, equipment, or physical therapy
Request any copies of x-rays/films from radiology you wish to obtain for your records
