Investigators requesting permission to conduct research involving human subjects within or outside Children's Hospital must submit an application to the Committee on Clinical Investigation (CCI).

The application should be completed electronically and unstapled, single-sided paper copies should be brought to the CCI office. The protocol application is located on Forms page of the website. All protocol applications must include:
|
- Parts A-D are required.
- Parts E-M should only be completed if they apply to your particular research.
- Principal investigator, departmental and scientific review signatures.
- Scientific Review Forms plus correspondence.
- See Scientific Review for additional information.
- If applicable, grant applications, investigator brochures, other supplemental materials.

- The complete, original application, plus one copy of the entire application, should be delivered to the CCI office.
- 3 copies of the grant application, investigator's brochure and other supplemental material should also be submitted.
- The electronic version of the Informed Consent document should be emailed to your CCI Administrator.

Depending on your protocol, additional reviews may be required.
