We suggest that all patients follow the guidelines below to obtain the correct referral.
A referral is a form or number from your child's primary care physician approving your child's visit to the Department of Plastic Surgery. Referrals can vary in the number of visits they cover, the length of time that visits can be used, and the services covered.
Referrals for each specialist
During your visit, your child will meet with many specialists. When you book your appointment we will provide you with their names and provider ID numbers. Please obtain a referral for each specialist prior to your visit.
Make sure you obtain a referral for each specialist your child will be seeing.
