The inpatient hematopoietic stem cell transplantation (SCT) team at Children's Hospital Boston consists of an SCT attending, one primary first-year fellow, 1 or 2 fellows who share night-time call, one nurse practitioner, a pharmacist and one junior resident who spends a two week, call-free block on the transplant unit. There are 13 designated transplant beds and there are occasionally transplant patients on other floors (primarily the oncology floor) who are covered by the SCT team. The resident provides care for patients newly admitted for autologous or allogeneic transplants, as well as for patients readmitted for complications following SCT. The resident divides the patient list with the nurse practitioner and is supervised by the fellow and attending. Residents provide the primary care for their patients including pre-rounding, presenting on rounds, writing daily notes, reviewing all radiologic and pathologic studies and running weekly family meetings. The rotation includes daily morning teaching sessions, and the resident gives one 20-30 minute presentation to the team on a topic of his or her choice. Residents also attend a weekly SCT staff development conference, the Longwood Medical Area SCT conference, and "sign-out" of patients at Dana-Farber. Rotation goals for the resident include an understanding of the indications for SCT, donor selection, expected complications and the time they occur in the course of transplant, long-term effects of transplant, and the psychologic issues experienced by children and families undergoing intensive therapy and long hospitalization.
